Consumer Notice: Patient Rights, Health Records & Complaints
In accordance with Texas House Bill 4224 (Texas Health and Safety Code Section 181.105), this notice provides step-by-step instructions on how to request your medical records, contact our state licensing board, or file a consumer complaint.
1. How to Request Your Health Care Records
You have the right to inspect or obtain a copy of your health and medical records maintained by Halcyon Village. To exercise this right, please follow these steps:
Step 1 ⇒ Obtain or Write a Request:
Prepare a written request. You may ask our front desk for a standard Records Release Form, or submit a written letter. Your request must include: your full legal name, date of birth, contact information, a description of the specific records requested, and your signature.

Step 3 ⇒ Processing & Delivery:
Our office will process your request in accordance with Texas state law timeframes. We will notify you when records are ready for secure electronic delivery or physical pickup. Note: A reasonable, cost-based fee may apply for copying or printing records as permitted by law.

3. How to File a Consumer Complaint
If you believe your consumer or privacy rights regarding your protected health information have been violated, you have the right to file an official complaint with the state.
To file a complaint against a provider/clinic: You can submit a formal grievance directly through the Texas Board of Chiropractic Examiners enforcement page online at www.tbce.texas.gov/place-a-complaint/.
To file a general consumer protection complaint: You may contact the Consumer Protection Division of the Office of the Attorney General of Texas, which maintains information under Section 181.103:
