Consumer Notice: Patient Rights, Health Records & Complaints

In accordance with Texas House Bill 4224 (Texas Health and Safety Code Section 181.105), this notice provides step-by-step instructions on how to request your medical records, contact our state licensing board, or file a consumer complaint.

1. How to Request Your Health Care Records

You have the right to inspect or obtain a copy of your health and medical records maintained by Halcyon Village. To exercise this right, please follow these steps:

Step 1 Obtain or Write a Request: 

Prepare a written request. You may ask our front desk for a standard Records Release Form, or submit a written letter. Your request must include: your full legal name, date of birth, contact information, a description of the specific records requested, and your signature.

Deliver your completed, signed written request to Halcyon Village via one of the following methods:

Step 3 ⇒ Processing & Delivery:

Our office will process your request in accordance with Texas state law timeframes. We will notify you when records are ready for secure electronic delivery or physical pickup. Note: A reasonable, cost-based fee may apply for copying or printing records as permitted by law.

3. How to File a Consumer Complaint

If you believe your consumer or privacy rights regarding your protected health information have been violated, you have the right to file an official complaint with the state.

  • To file a complaint against a provider/clinic: You can submit a formal grievance directly through the Texas Board of Chiropractic Examiners enforcement page online at www.tbce.texas.gov/place-a-complaint/.

  • To file a general consumer protection complaint: You may contact the Consumer Protection Division of the Office of the Attorney General of Texas, which maintains information under Section 181.103:
